This Privacy Policy describes the types of personal information collected by the Mental Health Association of Columbia-Greene Counties, Inc. ("MHACG") how we use such information, and to whom and under what circumstances we may disclose it.
This Privacy Policy applies to MHACG’s website, www.mhacg.org (the “Site”) and any personal information obtained when you call, email, or otherwise communicate with MHACG. By accessing the Site or otherwise interacting with us, you agree to this Privacy Policy. We can modify this Privacy Policy at any time, and will post the current version on the Site. We encourage you to periodically review our Privacy Policy to stay informed about how we are using the information we collect.
As a healthcare service provider, we ensure compliance with Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA) with our Notice of Privacy Practices available below and upon request.
Information We Collect When You Visit The Site

For each visitor to our Site, we may collect information such as the domain name of the website the visitor came from, the e-mail addresses of those who post messages to our contact form (if given), the e-mail addresses of those who communicate with us via e-mail, aggregate information on what pages visitors access or visit, information volunteered by a visitor, such as survey information, participation in chats or discussion boards, and/or site registrations. MHACG does not sell or rent your information for commercial purposes.

We may collect this information and use it for marketing and communications purposes, internal review, research, and to improve the content of our Web page, but personally identifiable information is generally not shared with other organizations for commercial purposes.

Mental Health Association of Columbia-Greene Counties, Inc. treats donor information that is collected offline in the same manner as it treats information collected online. Donor payment information may be collected through third-party processing services. MHACG does NOT endorse companies, products or services and strictly prohibits the suggestion of endorsement, recommendation, or superiority of one company, product or service over another company, product or service.

With respect to security: We endeavor to make sure that our Site and services provided by MHACG protect personal information that we receive are secure. When we transfer and receive certain types of sensitive information such as financial information, we redirect visitors to a secure server and will notify visitors through a pop-up screen on our Site. We are required by law to protect the privacy of health information that may reveal your identity, and to provide you with a copy of this notice which describes the health information privacy practices of our program staff and affiliated health care providers that jointly provide health care services with our program.
We are also required to notify you following breach of unsecured health information. A copy of our current Notice of Privacy Practices will always be posted in our reception area. You or your personal representative may also obtain a copy of this notice by requesting a copy from our program staff.
