January 2016 – Care Coordination

Care Coordination is a person-centered approach in facilitating individual health care needs for any person who is Medicaid eligible regardless of Diagnosis, as long as they meet Health Home Criteria. Operating with a staff of seven and providing services to both Columbia and Greene Counties, Care Coordination is a rapidly growing service in the agency as we move into the managed care system.

Care Coordinators in Columbia County are located at the Mental Health Association’s (MHA) main office location, and in Greene County at the Greene Medical Arts building (GMA). Both locations are easily accessible. The main office provides access to other MHA programs and services. The GMA location also has multiple providers and a pharmacy, where a face to face contact with a client is convenient for the Care Coordinator and the client.

Contracted through our Health Home (Hudson River Health Care), Care Coordinators are a team of Providers that serve clients in many ways. Care Coordinators help those who qualify attain the best possible services for physical and/or mental health. They can schedule appointments, advocate for services, and coordinate services for food, clothing and housing, assist in scheduling transportation for appointments and much more. They work closely with clients while developing a relationship that encompasses compassion and assisting clients in working toward their individual life plan. By linking clients to a multitude of services, monitoring their care and being their advocate, Care Coordinators help them manage their overall care with dignity and respect
